3 Jaw Micro-Vascular Approximator Clamp
3 Jaw Micro-Vascular Approximator Clamp
3-Jaw Microvascular Approximator Clamp
Patented. Peer-reviewed. Trusted across 500+ surgeries.
The Shira 3-Jaw Clamp is the only microvascular approximator clamp in the world with three independently spring-loaded jaws. It is the result of translational research at IIT Madras, validated in three peer-reviewed publications, and used at leading microsurgery centres including AIIMS, Apollo, Fortis, Max, Medanta and Aster.
What surgeons experience with conventional 2-jaw clamps
- Vessel wall folds inward (backwalling), obscuring the lumen
- Clamp must be flipped to suture the posterior wall — adding time and risk
- Adventectomy required to prevent tissue entrapment
- Anastomosis takes 15–45 minutes on average
- OT light reflection off the clamp reduces visibility
- Vessel slippage from clamp jaws
How the 3-Jaw mechanism solves each problem
| Problem | 3-Jaw Solution | Surgical Benefit |
|---|---|---|
| Backwalling / lumen obscured | Three jaws evert the vessel wall, keeping the lumen fully open | 0% backwalling — confirmed in RCT (vs Acland) |
| Posterior wall suturing requires clamp flip | Everted lumen gives direct posterior wall access | Clamp flip eliminated — no position change needed |
| Adventectomy needed | Third jaw grips adventitia instead of trimming it | Adventectomy not required — preserving adventitia accelerates healing |
| Anastomosis time | Faster set-up, no flip, no adventectomy | 2× faster — 35% faster in veins, 50% faster in arteries vs Acland |
| OT light reflection | Black anti-reflective coating on all jaw surfaces | Zero glare — easy to identify the clamp under microscope |
| Vessel slippage | Micro-grit gripping surface on jaw faces | Secure grip with less applied pressure — reduced intima trauma |
Clinical validation
- RCT: anastomosis time reduced by 35% (veins) and 50% (arteries) vs Acland — p<0.001
- Trainee study: 0% backwalling vs 30.8% with Acland; 0% clamp flip needed vs 38.5% with Acland; p=0.007

Engineering specifications
- Calibrated closing pressure — sufficient and consistent grip without over-compression
- Black anti-reflective coating on all jaw surfaces
- Micro-grit gripping surface to prevent vessel slippage
- Available in multiple sizes (2VV, 2AA, 3, HD) for vessels from digital arteries to radial artery scale
- Fits Shira Clamp Sterilization Case (7-clamp capacity)
- Compatible with Shira Clamp Applicator (asymmetric jaw design required for 2VV/2AA sizes)
Vessel size guide
- 2VV / 2AA — small vessels (digital arteries and small perforators)
- 3 — medium vessels (common free flap pedicles)
- HD — large vessels (radial artery, larger pedicles)
If there is no adventitia
The 3-Jaw Clamp can be used exactly like a 2-jaw clamp when the vessel has minimal adventitia — no technique change required.
